Families who had their child/ren visit the dentist in Term Two have already been contacted to confirm this Term Four treatment, or to opt out for this second treatment. Please ensure you have responded.
If your child/ren missed out last time Semper Dental are happy to facilitate an appointment for next week. However, you will need to complete the consent form and return it to the office by 9am on Monday 10 November. Printed consent forms are also available from the office.
You will be provided with a written copy of the dentist’s treatment plan for your child at every stage of the treatment process.
